How can one write code that allows a password change in Moodle to automatically be reflected in a password change in Joomla? Is it possible to have common database for both?

This is impossible because Moodle and Joomla salt hash passwords using different methods, so to retrieve the plaintext password for changing into a Joomla style password from a salted hash in the Moodle database is impossible.

Your only alternative is to have a 3rd seperate service which is used to change the passwords, which changes to both Moodle and Joomla at the same time.
